Specialized geriatric consultations at Jeffery Hale
Jeffery Hale can help you maintain your autonomy and stay in your home. Our Specialized Geriatric Consultations Department (formerly known as the Day Hospital) provides outpatient medical and nursing care as well as rehabilitation services.
Specialized geriatric consultations are for people aged 65 or over who are experiencing a loss of autonomy due to physical or psychological health problems and who require medical or nursing care or rehabilitation services. Seniors visit the Specialized Geriatric Consultations Department one to three times a week, depending on their treatment plan.
The following professionals specializing in geriatrics and geriatric psychiatry make up our consultation team:
- Physician
- Nurses
- Occupational therapist
- Physiotherapist
- Rehabilitation therapists
- Nutritionists
Referrals
Seniors must be referred to the Specialized Geriatric Consultations Department by a healthcare professional such as a doctor or social worker. Priority is given to people who are being discharged from a rehabilitation unit or a multipurpose bed in the public health system.
Transportation
Low-cost transportation to the department by minibus can be arranged through CIUSSS de la Capitale Nationale. Call us for more information.
Hours
The Specialized Geriatric Consultations Department is open by appointment from Monday to Friday, 8 a.m. to 4 p.m.
Location
The Specialized Geriatric Consultations Department is located between the chapel and the cafeteria. Take the stairs or elevator down to Level G.
