Step 1: Choose your testing centre 

Find the test centre nearest you by entering your postal code or consulting the map.

Step 2: Fill out the registration form

Once you have selected your test centre, fill out the online form to provide the required information before your test. Please note that you need to fill out the form for appointments and walk-ins. 

Book an appointment:

  1. Click on "Select an Appointment for a Test." 
  2. Choose the date and time.
  3. Fill out the registration form.

Walk-in: 

  1. Click on "Preregister without appointment."
  2. Fill out the registration form.

Conditions to undergo a screening test

Complete a self-assessment of symptoms

For a recommendation on the steps you need to take based on your condition, complete the COVID-19 self-assessment of symptoms.

You can undergo a screening test if you are in one of the following situations:

  • You have symptoms associated with the flu, gastroenteritis (stomach flu) or COVID-19 (fever, cough, difficulty breathing).
  • You do not have symptoms, but you have been in close contact with someone who has tested positive for COVID-19.
  • Public health officials have asked you to take a screening test.
  • If you are a traveller, please read Instructions for travellers during the COVID-19 pandemic.

Good to know before going to a test centre

COVID-19 screening tests are free for everyone. 

If you have a health insurance card, bring it with you.

If you do not have a health insurance card or any identification, you can still get tested.

      Request for certificate confirming isolation period

      The Public Health Department of the CIUSSS de la Capitale-Nationale does not issue certificates confirming that someone needs to be in isolation.

      To prove to your employer or insurance provider (or other) that the CIUSSS de la Capitale-Nationale does not provide certificates confirming the need for a period of isolation, present the following document in printed form (in French only) or on a mobile device.

      Disclosure of test results

      Negative results are sent by text message, email or telephone depending on the mode of communication selected by users during the screening test. If you have chosen email communication, be sure to check your spam folder to ensure receipt of the message containing your test result.

      Positive results are given over the telephone by the Public Health Department. If you have tested positive, you will receive instructions by email from the Public Health Department of the CIUSSS de la Capitale-Nationale.

      If you are named as a contact by someone who has tested positive, you will also receive instructions by email from the Public Health Department of the CIUSSS de la Capitale-Nationale.

      Request for the disclosure of a COVID-19 test result 

      If you have not received your test result after 7 days:

      • Fill out the request form below,
      • or call 418 821-0267,
      • or 1 833 518-0360 (toll-free number).

      We ask you to please wait the recommended time before phoning or submitting a request. You must use only one method of communication (online form or voicemail) and only if you have not received your result after 7 days.
